All volunteers who wish to participate in league events and be on the field for games and/or practices must print/complete the Holden Baseball or Softball volunteer form below, and mail it to the PO Box address indicated on the form. Please do not email us your forms. Please note, if you are a new volunteer, you also need to include a photocopy of your state issued ID.

IMPORTANT: After the form is received and processed by our coaching coordinator, an email link will be sent to allow prospective coaches to complete the required national background check electronically.

If the electronic background check process is not completed by the coaching candidate, the candidate will NOT be approved. You must complete/send the paper form -AND- complete the electronic background check process once you receive the email link to do so.
